Be aware that not all the places listed are hookah lounges exclusively. Some are coffee shops or restaurants with Middle Eastern cuisine and flair.  They cater to Middle Eastern clientele and are as intimate as a livingroom with a group of regulars, and YOU, as a newcomer, are expected to be on your best behavior. You know a place is good when you see Middle Eastern people. Don't drive them away with bad or loud behavior. ("We used to go there, but it has been overrun with disrespectful kids. They've ruined it for the rest of us.") In general, many hookah lounges are very classy and plush places with unique decor. Keep with the vibe, dress nice, be respectful and don't be a loudmouth.

Hookah Bar / Hookah Lounge owners:  Don't play Beastie Boys or Rage Against The Machine on the stereo and clean your hookahs often.

Hookahs belong on the floor and not on the table, because it is an object of service. Imagine the notion of "putting someone on a pedstal." This is along the same idea, and the shisha is an item of much lower rank, if you will. Traditional establishments will still adhere to this rule, while some hookah lounges are placing the hookah on the table for everyone to enjoy. Not very authentic, but it does celebrate the hookah. Smaller hookahs on a table are more convenient, as you can imagine.

Review:   Nice place, nice furniture, clean hookahs with screens and not foil, attentive staff, table games. Excellent pub food from next door is served, but no dining tables are in the hookah lounge. You pretty much eat off your lap or set your food on a low table or folding tray provided. Coffees, teas and other drinks also served, but no liquor. In a town that bans indoor cigarette smoking in public places, you may smoke here. Splash brand maassell (50g) sold to go as well as replacement parts for your hookah. Variable availability on flavors, but when they have them all, it's an impressive selection. Prices: $6.00 per bowl, $2.00 cover. Sunday Special: $3.00 cover, $3.00 bowls! A LITTLE-KNOWN SECRET: Arrive between 4pm and at least 15 mins before 8pm on a weekday. They have a happy hour special that coincides with the Devil House, the bar next door. Hookahs are only $5.00 if you get a pint next door for $2.00! (I think I'm the only one who knows.) You can't bring the beer into the hookah lounge, and you can't bring the hookah into the bar because of the Tempe smoking ordinance, so enjoy BOTH on the patio! A white peach hookah and a pint or two on the patio can't be beat! Pay and tip the outdoor bar server with the NEW DOLLAR COINS for a lark.Website

Cool middle eastern club music, sometimes regular rock hits, not too loud. Nice hoses for sale, but prices may be high. Finger coals in bags and self-lighting coals for sale at reasonable prices. A few shorter hookahs and traditional boxed tobacco (Nakhla) also for sale. Preferential treatment given to Arabic clientele. Nice looking shishas and hoses served, Nakhla brand tobacco is served. Large cake pans for coal trays, but that's OK given the crowded conditions which can bump coals, also for keeping your spare coals. Service is sparse on a busy night, it's a good idea to ask your server for whatever at once so your server doesn't make many trips to you - because he won't be able to! It's a good idea to arrange for one of your known things the moment you get your menu so you know something is in progress while you peruse the menu and wait for someone to get your dinner order. They're a busy bunch, I just think it's a good idea to contribute to their efficiency. The food is still as delicious and a very good value as I remember, and the place is still popular with students. So arrive early to avoid the crowds, as around dinner it gets crowded.

Their website shows a nice taller Egyptian hookah, but we don't know the actual sizes served. Flavors served include double apple, strawberry, grape, or mint. "DINE-IN ONLY," it says regarding the hookahs on the website, so I guess you're expected to eat something, and the food pics on the website look scrumptious! Nice chunky tomatoes. Mediterranean upscale lounge. That means no slobs. Happy hours are the best time to go, because of the pricing.
Wednesdays and Fridays are the days to go for half price hookahs! Wed. has happy hour pricing from 8-11pm and $2 appetizers!
Friday and Saturday nights are Greek nights, but Friday has hookah happy hour 4-7pm with happy hour pricing for drinks as well as half price appetizers.
Belly dancing and entertainment starts at around 9pm.
Nice looking hookahs but we haven't been there to smoke so we don't know how well they're prepared.
Comment from a local: "If you see the place is packed and there's a bus out front, don't bother going in because the waitstaff will be strained."
So, try to make it in when it's not crowded for better service.

Scottsdale Kebab
On Scottsdale Road, downtown Scottsdale near Stetson. Open till 10pm.
A small place on the east side of the street, this is a basic middle eastern restaurant with a very small bar up front which was surprising.
Outdoor sidewalk tables are available as well. On the cover of the menu are some pictures of nice tall Egyptian and Syrian hookahs, but in reality you're served a short, basic brass hookah with a basic wooden tip hose. I believe they serve Nakhla moasell. $7.99 for a hookah. You may smoke in the entrance foyer or outside on the sidewalk. If you go, it's best early evening toward sunset. Service can be spotty in frequency but very friendly and accomodating. Good portions at Scottsdale prices, but please understand that they have to pay for the Old Town Scottsdale rents.

This is a new pool hall that also serves hookah! Very relaxed atmosphere, it is casual and friendly. Has 11 pool tables, snacky type foods like pizza poppers and mozerella sticks, plus video games and air hockey. Music is a jukebox or whatever the owner is playing in his system if the jukebox is not going. TVs to watch sports, and a small lounge if hookah is your only interest. Nice new hookahs, and a large variety of tobacco to smoke, but none to buy as of yet.

On the south side of Apache in a strip mall at the Fire Station signal. Normal daytime hours only into early evening. Open later for Ramadan.
They have reasonable prices on Middle Eastern groceries as well as delicious hot food service.
Lots of Layalina and Nakhla (including Zaghloul) boxes, Al Jamay brand too.
The prices have gone up since hookah smoking has caught on in a big way, but deals can still be had if you're into Nakhla.
Curerntly as of end of Sept. 2007 they have natural coals on the bottom shelf and a few quick-light boxes.

Everything is in the aisle as you walk through the door.

This place should not be considered a cafe, the location is more like a small club with hookahs.
The atomosphere is loud and clubby, you have to raise your voice just to talk to the person across from you. Cover charge $5, a serving of hookah is another $5, 18+ only. Only non-alcoholic drinks are offered from a vending machine at the back. The clientele consists of all young people, sidways baseball caps love this place. The interior is plush with plenty of couches to lay on, although it does get crammed even on weekday nights. The 40" flat screen TV's are always bumping mainstream rap and trance music videos, this type of music is way too fast paced to enjoy smoking a hookah to. The service is fairly fast and friendly and the all-metal hookahs are well maintained. If you want a club type atmopshere that gets wild, this is the place for you. If you're looking for a place to relax and just want to enjoy smoking a hookah, try some more traditional smoking locations.

Alwaha is a mid-scale restaurant featuring Persian food and hookahs.
This site includes an interior eating area and a gated outdoor patio with a newly installed outdoor roof for hookah smoking.  Prices for food range from $5-$15 a dish and each serving of hookah is $7. Only non-alcoholic drinks are served. The clientele is a mixture of old traditional smokers to young sideways baseball cap wearing newbies. The inside consists of about 6 tables, and the outside has many smaller tables for smoking with a flowing fountain in the center as a nice touch. There are heating lamps outside if it gets too cold. Traditional Arabic (somtimes Arabic techno) music is somtimes played, but not enough of the time. The hookahs are mostly glass type, they are not so well maintained, half of the time they hit too harshly. The service can be slow and unattentive depending on the server. This is an average all-around lounge, if the service and hookah quality were improved it has the potential of being even better.

This is a new lounge that has just opened up in Anaheim, very close to Carthage and Hidden Café.
One big open room, soft couches along the walls, big screen TV in one corner playing belly dancer videos,
hip-hop music. Very good smoke, my mint was superb.

Good coal service. Good service all around.
The shishas were the tall 30" kind with very large bowls and tasted excellent.

They don't mix their fruit flavors with their mint or grape and they keep the bowls, hookahs, and hoses color coded as not to mix the flavors with subsequent uses. The decor is very simple, couches along the walls with little tables, dark colored walls (blue, purple), one wall is padded with red pillows and has three of those lighted mirrors on it. 
You can see the whole place from wherever you sit. The music was easy to talk over, they probably turn it up for the belly dancer. Clientele is mostly young people, no older traditionals in sight.
Other notes: Clean flavor coded hookahs, hot sandwiches on offer, no alcohol.
Dancing with the DJ! There's been a rise in young white kids, good time at night if that's your crowd.

D'vine has a large variety of shisha flavors and they'll let you smell them or reccommend one if you're not sure what to pick. We enjoyed this place so much our first visit, we came back the next night... and if you get addicted (to the place, not the shisha) too, they offer 6th one free for repeat customers. GREAT place!

It’s pricey to be sure; but this is Evanston, the center of suburban Chicago north shore - a high-rent district. It opens at 5:30 PM, and has a shisha special for $10 before 7 PM. Normal prices are 15$ for a bowl, with two coals. Refills are $7.00 and extra coals are $3.00 each. KA-CHING! This café also offers shisha pipes fitted with fresh fruit in the place of the traditional clay bowl; watermelon, cantaloupe, apple lemon, orange are available for $20-$30. Décor is nice, but not spectacular. Pipes are served on top of tables due to space, so traditional shisha manners are optional (the sideways wearing baseball set will be comfortable) and it’s BYOB. They also serve the normal Middle Eastern cuisine and tea. It’s crowded after 9pm, reservations are taken and recommended. Tables in the back are a little better, as the entrance can be seasonally cold. Parking meters are hungry and the eager Evanston police ticket hard, checking your meter every 15 minutes.

The hookah menu features 25 flavors of tobacco for $8 a bowl. The portion of tobacco is generous with about an hour's worth of smoking.
The staff is courteous, polite, and attentive. Extra charcoals are given with the hookah so you don't have to bother them for a replacement.
They have a big screen tv with arabic music videos and the decor is styled like ancient Egypt. It also features an Egyptian gift shop with statues and jewelry. Some nights feature bellydancing. An Egyptian cook is on staff and they serve Egyptian food and desserts including baklava, basbousa, konafa, and pyramid cake. They also have a wide variety of drinks including Turkish coffee, espresso, Nescafe, exotic teas, and fruit drinks. Alcohol is not served or permitted to be brought inside. Weekly specials feature Egyptian food such as shwarma, felafel, hummus, and gyros. They also have games for customers to play including backgammon, dominos, and chess. The crowd is a healthy mix of Middle Eastern Americans and college students.

From what I have been told, the OPM Club in the Caesars forum also has hookahs, as well as Habib's Middle Eastern Restaurant. There is also a restaurant called Layalina (spelling unsure) that has hookahs. In addition, there are two other hookah lounge/cafe/bars under construction. One is called the Hookah Cafe (across from UNLV) and another one in Silverado Ranch (a 'burb' of Vegas on the Southside). I don't remember the name of the one in Silverado Ranch, however, I met the Owner and she said the name of the place meant 'diamond' in Arabic.
